These forms are completed through the Parent & Student Portal, where they're saved, submitted, and reviewed by the school office.
- Student Health History Update
Annual health history and conditions checklist for the school nurse.
- Athletic Packet
Athletic participation release, NYSED Interval Health History, and Sudden Cardiac Arrest acknowledgment, plus uploads of a signed physical exam and current immunization record.
- Technology Forms
Chromebook registration or lease agreement, plus the Google Account Agreement.
- Transportation Request
Request public school district bus transportation to and from The King's School.
- Transcript Request
Request official transcript copies be sent to colleges or other recipients.
- Dental Health Certificate
Optional dental health assessment consent, plus upload of the dentist-completed certificate.
- Medical Authorization
Parent authorization for the school to administer medication, plus upload of each prescriber-signed order.
- Early Enrollment / Re-Enrollment
Re-enroll a returning student for next school year and sign the Financial Information sheet.
