Complete the Enrollment Packet
New families can fill out everything online in one sitting. Returning families re-enroll in the Parent & Student Portal.
Step 3
Set Up Tuition & Aid
Contact the business office to set up a FACTS payment plan, and apply for FACTS Grant/Aid if you need help.

New Student Forms for Enrollment
New families can complete the Application, Family Information, Student Information, Standard of Conduct, Transfer of Records Release, and Financial Information together in one online packet — no printing needed.
- How to Enroll
Checklist of everything to complete before the first day of school.
- Early Enrollment / Re-Enrollment
Early re-enrollment is $200 by May 1st; $300 after; $400 for new students.
- Application
Kindergarten–12th grade application. A $400 new student fee is due with it.
- Financial Information
Tuition, fees, billing, and policies for 2026–27.
- FACTS Grant/Aid Application
Apply for tuition assistance through FACTS.
- Family Information
- Student Information 5th–12th
Filled out by the student.
- Standard of Conduct — Elementary
- Standard of Conduct — 7th–12th
- Transfer of Records Release
Releases your child’s records from their previous school, including any 504s and IEPs.
- Student Physical Form
Completed by your child’s physician. Upload or turn in to the school nurse.
- 2026-27 NYS Immunization Requirements for Entrance
- Required NYS School Health Exam Form
Completed by your child’s physician.
- Medication Authorization
Parent authorization plus the prescriber-signed order for each medication.
- Student Health History Update
Returning Student Forms
Returning families can re-enroll and sign the Financial Information sheet online from the Parent & Student Portal.
